Attorney IOLTA trust account agreement

At the time an attorney/law firm opens an IOLTA trust account at their financial institution of choice, the attorney/law firm is required to complete the Attorney IOLTA Trust Account Agreement and distribute as described below.

After filling out the form, please make two copies from the original and distribute as follows:

• Original – Held by the financial institution

• Copy 2 – Retained by the attorney
